Payment Options

At Milk Stories, we believe high-quality lactation and infant feeding support should be accessible, individualized, and transparent. Below, you’ll find the three payment options we offer. Visit costs are dependent upon payment type. Click each section to learn more.

All new clients must fill out a request form regardless of payor source.

Out-of-Pocket Self-Pay

If you are not using insurance, we are happy to work with you directly. Many families choose self-pay for the flexibility and individualized nature of care offered at Milk Stories. You’ll receive the same integrated, evidence-based support that blends lactation expertise with a whole-child occupational therapy approach.

Your visit will be tailored to your unique goals and needs — whether that’s helping your baby feed more comfortably, preparing to return to work, or starting solids with confidence.

Self-Pay Fees

We are happy to accept Cash, Credit Card, and HSA/FSA for payment at the time of service.

Initial Visit $285
Follow-Up Clinic Visit $185
Virtual Visit $185
Travel Fee per Home Visit* $75

*Home visits are subject to baby’s age, your location, and provider availability, and may have limited availability.

Will I get reimbursed for self-pay services by my insurance?

If you’d like to seek reimbursement from your insurance, we will provide you with a superbill — a detailed receipt including procedure codes and provider information that you can submit directly to your insurance company. Reimbursement is not guaranteed and depends on your out-of-network benefits.

Our providers are licensed as both Occupational Therapists and Lactation Consultants. Because of this dual credentialing, we are able to bill under both occupational therapy and lactation codes for either parent or baby, depending on the services provided in your session.

Treatment codes

Lactation

  • S9443 (Modifier 33)
  • 96110
  • 99404

Occupational Therapy

  • 97167 Occupational Therapy Evaluation (Modifier GO)
  • 97530 Therapeutic Activity (Modifier GO)
Questions to ask your insurance

Since every insurance plan is different, the best way to understand your coverage is to call your provider directly. We recommend asking:

  1. Do I have out-of-network benefits? (Some plans do not cover out-of-network services.)
  2. What is my out-of-network deductible? (This tells you how much you’ll need to pay before insurance starts reimbursing.)
  3. Do I have out-of-network benefits for lactation? (See treatment codes above.)
  4. Have I used all of my lactation visits? If so, can I request more?
  5. What is my out-of-network coverage for occupational therapy services? (See treatment codes above.)
  6. What is the process for submitting a superbill for reimbursement? (Each plan has different requirements, so it’s helpful to ask about their specific procedures.)
In-Network Coverage for Aetna, UHC, and more The Lactation Network

Milk Stories is able to provide in-network services for families with Aetna, United Healthcare, and other plans through our partnership with The Lactation Network (TLN). This can substantially reduce the cost of services for covered families.

What’s covered by TLN
  • Lactation care for breastfeeding, pumping, lactation management, and weaning
  • Bottle-feeding, feeding transitions, and developmental concerns as part of lactation support
  • Integrated occupational therapy and lactation care with Milk Stories providers
  • Home and clinic visits
  • Prenatal lactation consultations
Out-of-pocket fees

As dual-credentialed IBCLCs and licensed occupational therapists, Milk Stories offers a comprehensive, integrated approach to infant feeding and lactation. That additional expertise allows us to evaluate body regulation, feeding coordination, and developmental patterns that influence how your baby feeds. The OT fees reflect this specialized support and are not covered by standard lactation benefits. Upon request, we will provide a superbill for your out-of-pocket costs to submit to your insurance for occupational therapy services.

Initial Visit $135 · OT evaluation fee
Follow-Up Visit $35 · OT fee
Virtual Visit $35 · OT fee
Travel Fee per Home Visit* $75

*Home visits are subject to baby’s age, your location, and provider availability, and may have limited availability.

How do I know if I qualify for TLN?
  1. Fill out an assessment request form.
  2. Submit your insurance information through our TLN verification link. TLN will review your eligibility and email you directly with approval within 2–3 days.
  3. Once benefits verification is complete and a request form is submitted, we’ll do our best to contact you via text message and email within 2 business days.

You must verify your benefits prior to your appointment.

What plans are accepted by TLN?

Coverage with TLN may change, and as contracted providers we unfortunately have no control over those changes. As of May 2026, we are in-network for lactation benefits through TLN with:

  • Aetna Commercial Plans
  • Curative Health
  • Imagine Health
  • Meritain Commercial Plans
  • Multi-Plan PPO
  • PNOA
  • Sutter Health
  • United Healthcare
  • VA CCN Optum East
  • Velocity PPO

For the most current list or to determine your plan’s coverage, please fill out a verification form or reach out directly to TLN.

Will TLN cover virtual visits?

TLN will not cover telehealth or virtual visits for any UHC or Aetna plans. Some smaller plans may include coverage. Specific coverage type and visit limits (if any) will be sent to you and your provider upon verification.

Does TLN cover bottle-feeding or solids?

TLN covers feeding support only for lactating parents. If you’re still nursing or pumping, visits that include bottle transitions or solids guidance may be covered. If you’ve weaned or are exclusively formula-feeding, we’re still happy to work with you as a private-pay client.

In-Network Coverage for Cigna Wildflower Health

Milk Stories is able to provide in-network services for families with Cigna through our partnership with Wildflower Health. Many families with Cigna are eligible for visits without a cap when medically justifiable. This greatly reduces the cost of sessions for families.

  • Integrated occupational therapy and lactation care with Milk Stories providers
  • Virtual, clinic-based, or home visit options
  • Prenatal lactation consultations
Out-of-pocket fees

The first visit includes an occupational therapy evaluation to support whole-child feeding and development. This approach takes additional training, licensure, and time.
